Thank you for becoming a patient of Gastro Health. Please arrive 30 minutes prior to your scheduled time to ensure all paperwork is complete before your appointment. Additionally, please bring with you a copy of your photo ID, insurance card(s), a referral issued by your primary care physician (if required by your insurance), a list of medications you’re taking, and any test or medical results that pertain to your visit. We look forward to seeing you!

Insurance and Billing
We accept most major insurance companies. We recommend you either call your insurance company or visit its website to verify if we participate with your plan. When you call to schedule an appointment, we will also be happy to confirm if we participate with your insurance.
We file all claims on your behalf. Copays are expected at check in. If you are being seen for a procedure, every effort will be made to inform you of your portion of the cost, which is due on the day of the procedure. If it becomes necessary to bill you for the balance, you will be billed within a month, and payment is due upon receipt.

We are located in the Woodbridge Professional Building (near Envoy Nursing Home) and across the street from the Nissan Dealership. If entering throught the back door entrance, our office is the 1st door on the left (Suite 103).
