  • Current Medication List with dosages (including over the counter supplements)
  • Relevant labs or diagnostic imaging
  • Referral (if required by insurance)
  • Insurance Card and Picture ID

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctor. We look forward to your first visit.

Insurance and Billing

We accept most major insurance companies, including Aetna, Anthem, Blue Cross Blue Shield, Care First, Cigna, and United Healthcare. We recommend you either call your insurance company or visit its website to verify if we participate with your plan. When you call to schedule an appointment, we will also be happy to confirm if we participate with your insurance.

We file all claims on your behalf. Copays are expected at check in. If you are being seen for a procedure, every effort will be made to inform you of your portion of the cost, which is due on the day of the procedure. If it becomes necessary to bill you for the balance, you will be billed within a month, and payment is due upon receipt.

Payment Options

New Patients need to pay with credit card or cash.
Established Patients may pay with credit card, cash, or check.


Driving Directions

We are located in the Woodbridge Professional Building (near Envoy Nursing Home) and across the street from the Nissan Dealership. If entering throught the back door entrance, our office is the 1st door on the left (Suite 103).
